A Gangliocytic Paraganglioma of the Ampulla of Vater Treated by Endoscopic Snare Papillectomy

Abstract
Gangliocytic paraganglioma is an extremely rare tumor that is usually seen in the duodenum. It has unique histologic features composed of endocrine cells with a carcinoid-like appearance, isolated ganglion cells, and spindle-shaped Schwann cells. Generally, this tumor is considered benign, because only a few cases of regional lymph node metastases and local recurrence have been reported. But in most patients, resection, including pancreatoduodenectomy, was performed because preoperative diagnosis was difficult. We present a case of a gangliocytic paraganglioma of the ampulla of vater misdiagnosed as ampullary adenoma and treated with endoscopic papillectomy. The resected specimen revealed a gangliocytic paraganglioma. All margins of resection were negative for tumor. A size of the specimen was 2.0¡¿1.3¡¿1.0 cm. We present this case, because gangliocytic paraganglioma is a rare tumor, and it was successfully treated with endoscopic resection.
